Administration of Medication during school hours:
The purpose of administering medication during the school day is to help each child maintain an optimal state of health that may enhance his/her educational opportunities. Medication shall be administered during school hours that are necessary to provide that student access to the educational program.
All medication, including prescription and non-prescription (over the counter) drugs, shall require a written order from the physician. All medication shall be provided by the student's parent/guardian. All medication shall be stored in and administered from the Health Office.
The appropriate form can be found on the Medication Authorization form section of the Medical Info drop down menu on the Middle School main page.
Medical Requirements
1. All students entering Kindergarten will be
required to have a physical** with the age appropriate immunizations
2. All students entering an Illinois School for the first time will be required to have a physical** with age appropriate immunizations.
3. All students entering 6th grade must have a new
physical** with up to date vaccines including
a. A tdap
vaccine
b. Menningitis
vaccine
c. 2(two)
varicella vaccines or physicians documentation of history of chickenpox.
